MDS 3.0 Basics; Revisiting Section M: Skin Conditions

This section, which is rich with information for the care planning team, has significant implications for survey, payment and publicly-reported information. Because of the significant regulatory and legal implications of assessment related to skin, education and training of staff and assurance of competency in skin assessment, including measuring and staging pressure ulcers is a must. The RAI User's Manual reminds us, "(i)t is imperative to determine the etiology of all wounds and lesions, as this will determine and direct the proper treatment and management of the wound."

CMS Stresses Accuracy on MDS Interviews

Today’s Medicare Minute we are going to talk about interviews on the MDS.  At the MDS National Convention CMS stressed the importance of interview accuracy on the MDS.  Unlike the MDS 2.0, MDS 3.0 has four structured interviews.  This includes the PHQ-9, which looks at the patient’s mood, The BIMS, which looks at the patient’s cognition, Activity preferences, and pain.  Each one of these interviews has a specific timeline in which they need to be completed.  The PHQ-9 should be honored before the ARD, the BIMS has a seven day look back, the activity preference interview has a seven day look back, and the pain interview has a five day look back.  In completing your interviews, you want to make sure you read the RAI Manual carefully, including Appendix D, because there are great tips and guidelines to make sure you get the most out of your interview process.   If you are signing sections that have interviews you might have to sign the MDS twice, once before the ARD for the interview and again after the ARD for the rest of the section.  Whatever your process is, you want to make sure that you have good systems set up so that not only are you getting the most out of your interview, but that you have got ARDs well managed so that interviews are timely.

CMS Posts MDS 3.0 RAI Manual Updates.......Again

Fasten your seatbelts as there are 6 days left (yes, only 6 days) until the Final Rule changes to the SNF PPS and RAI process go into effect.  Following a historical pattern, CMS posted additional changes and updates to the MDS 3.0 RAI manual on September 20, 2011.  The MDS 3.0 Training Materials web page states: Files that have changed are Chapter 2, Chapter 6, and Chapter 3, Section O. Previous files are not completely accurate and should be replaced with these immediately.

MDS 3.0 RAI Manual Changes

The Resident Assessment Instrument (RAI) User's Manual has been updated to reflect changes that take effect on October 1, 2011, the manual version is 1.07 (V1.07). Providers should take extra care to ensure that materials used after October 1st reflect this version of the RAI. Several chapters and appendices include only minor changes and edits however there are several significant changes that providers should make note of. Harmony has outlined some significant updates from Chapter 2 below and encourages clients to contact either their Harmony Consultant or the Corporate Office with any questions, including those regarding how to operationalize the changes.

CMS Publishes MDS 3.0 RAI Manual Updates

As of June 2, 2011 CMS posted multiple clarifications to the current MDS 3.0 RAI manual.  The CMS site which hosts pdf  files of the manual and MDS 3.0 subsets now contains newly updated Chapters and Appendixes.
