Site Visits
Harmony Healthcare provides a unique service that assists the long-term care facility with ensuring that the Medicare Program is appropriately managed. Through full day site visits, Harmony audits the PPS/MDS prior to CMS transmission to ensure accurate reimbursement. Within these full day site visits, Harmony also educates and trains the Medicare Team on Medicare guidelines, updates and regulatory interpretation. Services include the following:

  • MDS/PPS Assessments: Auditing documents to ensure accuracy and optimal reimbursement while simultaneously educating the team to case management techniques through formal and informal training.
  • Part A Coverage: Assisting the facility (on-site and off-site) in making appropriate coverage determinations on Part A beneficiaries through on-site chart review and/or telephone/fax review.
  • Medicare Manual: Each facility receives a Medicare Manual Volume I to reference throughout the course of the contract.
  • Reports: A detailed site visit report to follow every monthly visit describing each patient reviewed, areas of opportunity and actions to be completed. Analysis also incorporated into the report, i.e., RUG's mix discussion, Part A rate trending, etc.
  • Customized Training: Site visits include any customer-requested training for the facility staff on-site. For example, Harmony performs "Medicare Nursing Documentation" training in one-hour slots to accommodate the nurses on the unit.
  • Analysis: At the facility's request, Harmony to complete financial/operational analysis of Medicare and therapy related areas (i.e., financial impact converting therapy from contract labor to in-house, or vice versa).
