Interview with Heath Boddy, Executive Director of the Nebraska Health Care Association

Kris Mastrangelo, President and CEO at Harmony Healthcare International (HHI), interviews Heath Boddy, Executive Director of the Nebraska Health Care Association, in the AHCA Provider Lounge.   Heath discusses the pros and cons of the Five-Star Rating.  While Heath believes transparency is a good thing, he finds the rating system to be flawed in several aspects.  First off, the Five-Star Rating is on a bell-curve that requires at least 10% of the providers in a state to be one-star.  By this measure, regardless of the level of care provided all facilities in a state will never all be four or five stars.  Additionally, Five-Star Ratings are state specific.  A Five-Star facility in Nebraska may not be comparable to a Five-Star facility in Iowa or vice-versa.

Kris Mastrangelo, President and CEO at Harmony Healthcare International (HHI), interviews Mark Parkinson, President and CEO of the American Health Care Association, in the AHCA Provider Lounge.   Mark discusses his views on PDPM, Five-Star and state Medicaid rates.  He says he, like most of the industry, is cautiously optimistic about PDPM as it brings focus back to nursing and restorative care.  He also shares his concerns about the current state of the Five-Star rating.  He would like to see CMS incorporate patient satisfaction into Five-Star.  Additionally, Mark and his team at AHCA have been working to help increase Medicaid to states that meet certain criteria.

Health Inspection: Five Star Impact

The Health Inspection Star Rating is calculated using the results of the last 3 years of onsite inspections and includes both standard surveys and any complaint surveys.  The most recent survey findings are weighted more than the prior two years.  Points are assigned to individual health deficiencies per their scope and severity. CMS assigns more points to facilities with deficiencies of greater scope or severity and recent surveys are weighted more heavily.

The Five-Star system has been updated (July 2016) to include 5 new measures in the calculation of the Quality Measure portion of Five-Star. A Five-Star Preview Report is available to facilities through the CASPER reporting system. The new reports now include point assignments for each of the 16 Quality Measures that calculate into a facility Five-Star Rating. The initial release of the Five-Star Preview Report contained errors in calculation.

SNFs Beware:  Your Five-Star Rating May Be Impacted on July 27th
