Accident and Incident Investigation
With ever increasing scrutiny and review of incident investigations, especially as they may relate to being state reportable incidents, it is important that providers clearly understand all aspects of incident investigations. Evidence of investigation must be readily available to state licensing and certification staff and others according to their authority. This documentation may be in the format and location selected by the provider and must contain information and facts that address “who, what, when, where, how and why” of the incident. The completion of a thorough investigation with a “root cause” analysis and review of systems are critical to minimize harm and prevent recurrence.
Harmony Healthcare International (HHI) has Licensed Health Care Risk Managers on their staff with focused training on completion of investigations and review of systems to analysis and manage risks. This focused review will include:
- Review the provider’s current policy and procedure related to the accident /incident reporting to evaluate if it supports the documentation of a thorough investigation, corrective action development and is consistent with regulatory standards
- A review of the incident report and related investigation to determine whether the root cause analysis was accurately documented
- Assist with the development of a corrective action plan that will effectively eliminate or mitigate the causative factors to prevent recurrence
- Provide assistance with periodic oversight of the corrective action plan to assess implementation and effectiveness
