Therapy Services (PT/OT/ST) rendered in SNF are subject to the parameters outlined in the Medicare Benefit Policy, Chapter 8.
Specifically, the daily skills, knowledge and judgement of therapist are necessary for the provision of said services as no other caregiver is qualified to render the interventions. The ultimate goals of skilled therapy are for the patient to:
- Attain his or her prior level of function,
- Achieve his or her highest practical state of wellbeing,
- Prevent deterioration, and or
- Establish and Implement a maintenance program.
Multiple clinical conditions may impact the patient’s response to therapy. The Requirements of Participation emphasize the expectation of person-centered care. This model of care permeates beyond the nursing domain and includes all aspects of care, including therapy. Treatment individualization requires ongoing refinement and adjustments to the therapeutic interventions. This may be seen in variations of the expected and actual treatment time, i.e., minutes of therapy. In fact, minutes rendered in a consistent pattern may be perceived as non-individualized. It is prudent to assess whether the therapy team is aware of this risk area and provide guidance on the regulation.
